How to enroll via fax, mail or online

When it is time for you to join, we will send you an enrollment packet in the mail.

You can enroll over the phoneonline and at community enrollment sessions, and you can also enroll by sending your form by mail or fax.

Fill out the enrollment form that comes in your enrollment packet. Then, you can fax it to us for free at 1-866-482-3676 (1-866-4U2Enroll).

    I was trying to re-enroll my daughter in Medicaid. What should I do?

      To reapply for Medicaid coverage, you will need to contact your county Division of Family and Children Services (DFCS) office. You can locate your local DFCS office by visiting http://dfcs.dhr.georgia.gov (Click on “County Offices” from the list on the left. When the map appears, click on the region that has your county in it).

    What am I expected to put in the password box when enrolling my id# on here?

      The first time you create an online profile on our website, you should use your initial password. The initial password was listed on the first letter you received from Georgia Families concerning enrolling. If you have misplaced this letter, please contact us in person to obtain this. We apologize for the inconvenience.

      Our Enrollment Counselors can be reached at 1-888-GA-Enroll (1-888-423-6765), Monday through Friday, 7:00 a.m. to 7:00 p.m., (TDD: 1-877-889-4424). (Please listen to all of the prompts before making a number selection. Select option #1 for Georgia Families, then option #1 for “Check Enrollment Status”. Then enter option #2 for “Using your Medicaid ID number”. When prompted for a Member ID, enter nothing. You will be prompted a second time to enter a Member ID number. Enter nothing again. You will then be transferred directly to an Enrollment Counselor.)
