IRS Form 1095-B

Did you know that as a part of the Affordable Care Act, an IRS Form 1095-B will be provided by Georgia Medicaid to all members early next year?

This form is used to report that you had health coverage through Georgia Medicaid during the tax year.  In order to ensure that you receive the form timely, you can consent to have the form delivered to you electronically through the Georgia Medicaid website.  If you choose to receive the form electronically, and you are already registered on the Georgia Medicaid website, please update your email address.  You can provide your consent and review a list of Frequently Asked Questions by going to www.mmis.georgia.gov under the member information tab.

Consenting to electronic delivery will ensure that you receive your form in time to assist you in filing your taxes for the upcoming tax year.
