Lost Medicaid or Health Plan ID Cards

It’s very important that each member of Georgia Families take two identification cards with them every time they go to see the doctor:

  1. Your Medicaid/PeachCare for Kids ID Card
  2. Your Health Plan ID Card 

These cards help Medicaid and your health plan keep track of the services provided to you, and your children.  Also, it’s very important that the information on these cards is correct and that you take them with you each and every time.

Did you lose your ID cards?

Well, we here at Georgia Families cannot mail you a Medicaid ID card.  Rather, these cards will be sent to you directly from Medicaid.  So, in order to get more information about replacing a lost Medicaid ID card, please call Medicaid’s Member Services at 1-866-211-0950

Also, if you need to replace your Health Plan ID card, just give your health plan a call and they will send you a new one quickly.

    Is there a way to request these documents online? It seems that it would be a lot easier than having to call a number where you may have to wait several minutes to speak with someone.

    After they correct the information, they will send us an update. We will then send that information to your health plan. At this time, you should request new health plan cards.

    I never received a peachcare id card for one of my children. How do I get a card?

    My auntie qualified for Medicaid and never got a card in the mail. Its been so hard to get the medicaid department to help send a new one. She qualified since July and has not gotten a card yet.
